Care Model
• We provide pediatric care through home visits and telehealth.
• We are not an emergency service. For emergencies, call 911.
• There is no walk-in clinic. All visits are scheduled.
Billing
Commercial Insurance (BCBS, UHC, Cigna, etc.)
• Insurance is billed first.
• You are responsible for copays, deductibles, and coinsurance assigned by your plan.
• A card on file is required for commercial insurance patients.
• Late cancellations and no-shows may result in a fee.
Self-Pay
• Payment is required before the appointment.
• A card on file is required.
• Late cancellations and no-shows may result in a fee.
Medicaid
• Medicaid is billed directly.
• No card on file is required as a condition of care.
• No cancellation or no-show fees are charged.
• We ask that families cancel as early as possible so another child can use that visit time.
Telehealth
• Telehealth works well for many common concerns.
• A reliable device and internet connection are needed.
• Some conditions require an in-home or emergency evaluation.
Communication
• Messages are answered during business hours.
• Text and portal messages are not monitored 24/7.
• Do not use messaging for emergencies.
Privacy
• Your child’s medical records are protected under HIPAA.
• We do not sell patient information.
• Clinical photos are used only when medically necessary and stored securely.
Teen Confidentiality
• Illinois law allows certain confidential services beginning at age 12.
• We support appropriate teen privacy while keeping parents informed within legal limits.
Accessibility
• Accommodations and interpreter services are available upon request.
• We work to ensure equal access to care.
